This should be filled out for new business opportunities that fit at least one of these criteria:

1. Anything that will trigger new procurement.
2. New manufactured item or packaging format.
3. Opportunity outside of the continental U.S.
4. Customer is a distributor or manufacturer we are not currently working with.
